How do I enroll?


Before you enroll: have you read our Evidence of Coverage?

Enrollment Period

The annual enrollment period begins November 15, 2007 for a January 1, 2008 coverage date.


Enrollment Options


Submit Enrollment Form

You can print an Enrollment Form from our website, or you can request a printed enrollment form to be mailed to you by calling Vantage at 1-800-823-1910 or 318-361-0900 (TTY/TDD 1-866-524-5144).Hours of operation are:

November 15th through March 2nd:
Seven (7) Days a Week 8:00 A.M. - 8:00 P.M.

All other Dates:
Monday Through Friday Week 8:00 A.M. - 8:00 P.M.

Eligibility

Beneficiaries who choose to enroll in a Medicare Advantage Plan (MA):

  • must be entitled to Part A and enrolled in Part B as of the effective date of coverage under the MA plan.
  • are still in the Medicare program
  • still have Medicare rights and protections
  • still get all Medicare-covered services

You May Qualify for Assistance

If you have limited income and resources, you may qualify for extra help with Medicare costs. To find out if you are eligible and how to apply, you can contact the Social Security Administration at 1-800-772-1213 (TTY 1-800-325-0778) or visit www.socialsecurity.gov.


If You Disenroll from Vantage in the Future

If you will be changing to original Medicare, you might have a special temporary right to buy a Medigap policy, also known as Medicare Supplemental Insurance, even if you have health problems. For example, if you are 65 or older and you enrolled in Medicare Part B within the past 6 months or if you move out of the service area, you may have this special right.

Federal law requires the protections described above. The state may have laws that provide more Medigap protection. If you have questions about Medigap or any special temporary rights you may have, you should contact the Senior Health Insurance Information Program (SHIIP) at 1-800-259-5301 or visit website www.ldi.state.la.us/Health/Shiip/index.htm to get more information about Medigap policies in this state.

If Vantage Terminates Its Contract with CMS

Vantage Health Plan, Inc. has a contract with the Centers for Medicare & Medicaid Services (CMS), the government agency that runs Medicare. This contract renews each year. At the end of each year, the contract is reviewed, and either Vantage Health Plan, Inc. or CMS can decide to end it. You will get 90 days advance notice in this situation. It is also possible for our contract to end at some other time during the year. In these situations we will try to tell you 90 days in advance, but your advance notice may be as little as 30 or fewer days if CMS must end our contract in the middle of the year.
